Job summary

Chatham Hills Subacute Care Center is seeking a Licensed Practical Nurse (LPN) for the day shift to care for residents in our Skilled Unit and Long Term Care Unit. You will administer medications, monitor vital signs, and provide education to residents and families as part of a collaborative care team.

Qualifications

• Current LPN license is required.
• Skilled nursing experience preferred.
• Ability to perform IV medication administration if permitted.

Responsibilities

• Provide care for residents in SNF and Long Term Care units.

• Take and record vital signs accurately.
• Deliver resident care and treatments with outcome monitoring.
• Educate residents and families about care plans.
