Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing and long-term care units. The role focuses on medication administration, vital signs, and providing comprehensive resident care with close monitoring of outcomes.

Responsibilities include coordinating with physicians and families, educating residents, and supervising CNAs to maintain high-quality care.

Qualifications

• Current LPN license in good standing.
• Experience in long-term care or SNF is preferred.
• Ability to administer medications, including IVs, and monitor outcomes.
