Job summary

The Monarch at Brooklyn Rehabilitation and Nursing Center seeks an LPN to join a growing team in a skilled nursing setting. You will provide resident care, administer medications, monitor vital signs, and educate residents and families.

An active LPN license is required, with experience in SNF/long-term care preferred. Competitive wages, union benefits, and comprehensive medical/dental/vision coverage are offered.

Qualifications

• Current LPN license and eligibility to work in New York.
• Experience in skilled nursing or long-term care preferred.
• Ability to administer medications and monitor patient outcomes.

Responsibilities

• Provide care for residents in SNF and Long Term Care units.
• Administer medications, including IVs when required.
• Take and record vital signs and monitor patient status.
• Educate residents and families on care plans.
• Communicate with physicians, NPs, families, and supervise CNAs.

Licensed Practical Nurse (LPN) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;


• Medication Administration, including IVs.


• Taking and recording vital signs.


• Providing resident care and treatments with monitoring of outcome.


• Provide resident and family education.


• Communication with physicians, NPs, families and other health care professionals.


• Supervising CNAs
