Job summary
The Monarch at Brooklyn Rehabilitation and Nursing Center is seeking an LPN to join our skilled nursing team in Hempfield Township, PA. The position involves caring for residents in both the Skilled Unit and Long Term Care Unit, with duties including medication administration, vital signs, patient education, and collaboration with a multidisciplinary team.

- Take and record vital signs.
- Provide resident care and treatments with monitoring of outcomes.
- Provide resident and family education.
- Communicate with physicians, NPs, families and other health care professionals.
- Supervise CNAs.

- Communication with physicians, NPs, families and other health care professionals.
