Job summary

St. Joseph Hospice in Lafayette, LA is hiring for full-time Licensed Practical Nurses (LPN) to join our patient-centered team.

This role focuses on compassionate, quality end-of-life care in a supportive work environment. You will provide nursing and supportive care under the direction of the RN, coordinate with physicians and hospice staff, document all services, and ensure compliance with Medicare/Medicaid hospice benefits.

Qualifications

• Louisiana LPN license is required.
• Minimum 2-3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
