Job summary

St. Joseph Hospice in Baton Rouge, LA is seeking a Full-Time Licensed Practical Nurse (LPN) to provide nursing care under the RN and interdisciplinary team. You will document services and ensure care aligns with the care plan.

Qualified candidates hold a Louisiana LPN license, 2–3 years of LPN experience, CPR certification, and reliable transportation. Hospice experience is preferred; ESOP ownership, competitive pay, disability coverage, PTO, and mileage reimbursement are offered.

Qualifications

• Louisiana LPN license required.
• 2–3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care under the direction of the RN.
• Communicates with attending physicians and hospice team to coordinate care.
• Documents services provided on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid hospice benefits regulations.
• Reports changes in patient condition to the RN.
• Supports ESOP ownership benefits and related company policies.
• Assists with mileage reimbursement and PTO processes.

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
