Job summary

St. Joseph Hospice in Alexandria, LA is hiring full-time Licensed Practical Nurses (LPNs) to provide compassionate nursing and supportive care under the direction of RNs, as part of an interdisciplinary team.

The role emphasizes accurate documentation, coordination with physicians and hospice staff, and adherence to Medicare/Medicaid hospice guidelines. Ideal candidates will have Louisiana LPN license, CPR, 2–3 years' experience, reliable transportation, and hospice exposure.

Qualifications

• Current Louisiana LPN license required.
• Minimum of 2-3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver's license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
• Great time management skills
