Job summary

Green Hills Center for Rehabilitation and Healing in Tennessee is seeking a Licensed Practical Nurse (LPN/LVN) for full-time or part-time roles with day and night shifts. You will provide skilled nursing care in SNF and Long Term Care units, monitor residents, and collaborate with physicians and families.

The position requires a current Tennessee LPN license, ability to supervise CNAs, and strong patient education skills; 12-hour shifts are available to support continuity of care.

Qualifications

• Current Tennessee LPN license.
• Experience in SNF/Long Term Care preferred.
• Willingness to work 12-hour shifts (day or night).

Responsibilities

• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
