Job summary

STAT Home Health in Houma, LA is hiring full-time Licensed Practical Nurses (LPN) to provide skilled nursing and supportive care in the home under the Interdisciplinary Plan of Care.

You will document services and coordinate with physicians and the home health team to ensure optimal patient outcomes.

Qualifications

• Louisiana Licensed Practical Nurse license is required.
• Minimum 1 year clinical experience as an LPN.
• Current CPR certification is required.

Responsibilities

• Provide nursing and supportive care under the Interdisciplinary Plan of Care.
• Coordinate with attending physicians, home health team, and other agencies.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition to the RN.

Job Duties:

• Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN

Qualifications:

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum 1 year clinical experience as an LPN
• Current CPR Certification

• Reliable transportation to conduct patient visits at their place of residence or facility
