Job summary

Magnolia Creek Facility is seeking a dedicated LPN to provide bedside care and support to patients across day and night shifts. The role involves recording vital signs, collecting health information, and coordinating with registered nurses and physicians to ensure safe, compassionate care.

Responsibilities include administering medications, testing blood sugar levels, catheter care, and maintaining accurate charting and records.

Qualifications

• Provide bedside care and personal hygiene support.
• Record vital signs and monitor patient status.
• Collect health information and relay to nurses/physicians.
• Administer oral medications and injections.
• Test blood sugar and manage catheter care.
• Maintain patient records with accurate documentation.

Responsibilities

• Ensure patient comfort and safety at all times.
• Communicate changes to supervising staff promptly.
• Adhere to facility policies and privacy regulations.
• Support LTC, geriatrics or rehab patients as needed.
• Assist with routine daily activities and mobility.

LPN responsibilities

• Provide bedside care to patients including personal hygiene maintenance
• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)
• Collect patient’s health information
• Relay information between patients, registered nurses, and physicians
• Give oral medications and injections to patients
• Test a patient’s blood sugar levels
• Urinary catheter insertion and care
• LTC, geriatrics or rehab.
• Assist patients with personal hygiene
• Monitor for changes in patients
• Proper charting, documentation, and maintenance of patient’s medical records
• Asks questions when in doubt for clarification by the Registered Nurse.
• Acts as a role model for ancillary personnel.
• Maintains the confidentiality of the patient’s clinical records.

LPN qualifications

• Up to date first aid and CPR certification
• Current LPN license in the state
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor
