Grant-Healthcare-and-Rehabilitation in Williamstown, KY is seeking an LPN to join our care team. You will provide bedside nursing, coordinate with the healthcare team, and monitor patients across a range of medical needs.

Qualifications

• Current LPN license in the state.
• Up to date first aid and CPR certification.
• At least one year of LPN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers prescribed medications and records them; reports adverse reactions per policy.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment used on patients and interprets electronic displays.
• Initiates corrective action when the patient shows adverse symptoms.
• Provides bedside care for a wide range of medical patients, including pre- and post-surgery.
• Prepares equipment and aids the physician during examinations and treatments.
• Responds to life-saving situations based on nursing standards and protocol.
• Participates in discharge planning.
• Uses time management skills and documents care concisely and accurately.

LPN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Report adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
• Records patients' medical information and vital signs.
• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.
• Initiates corrective action whenever the patient displays adverse symptomatology
• Provides bedside care for a wide variety of medical patients, including pre- and post-surgery patients.
• Prepares equipment and aids the physician during the examination and treatment of a patient
• Responds to life-saving situations based on nursing standards and protocol
• Participates in discharge planning.
• Utilizes time management skills.
• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
• Performs other position-related duties as assigned, depending on the assignment setting.

LPN Requirements:

• Current LPN license in the state
• Up to date first aid and CPR certification
• At least one year of LPN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred
