Job summary

Creekside Center for Rehabilitation and Healing in Brentwood, TN seeks a full-time Physical Therapy Assistant to support geriatric residents in a Skilled Nursing Facility. You will assist the PT, evaluate and treat independently, and contribute to residents' mobility, pain relief, and overall wellness. Required is a valid PTA license in the state.

SNF experience is preferred but not required. The role offers a stable schedule Monday–Friday with competitive benefits including insurance and fringe

Qualifications

• Must hold a valid Physical Therapy Assistant license in the state of practice.
• Ability to evaluate and treat independently without direct supervision.

Responsibilities

• Assist the PT with therapies designed to improve mobility, relieve pain, prevent or limit permanent physical disability and promote overall fitness and wellness in a skilled nursing facility.
• Develop and implement treatment plans in a SNF setting.
• Monitor progress and adjust therapies as needed.
• Document patient progress and communicate with the care team.
