Job summary

St Joseph Hospice in Baton Rouge, LA is hiring a Full Time Licensed Practical Nurse (LPN) to provide nursing and supportive care under the direction of the RN, following the Interdisciplinary Plan of Care.

The role requires current LA LPN license, 2–3 years of LPN experience, CPR certification, reliable transportation, and preferably hospice experience. We offer ESOP, competitive pay and comprehensive benefits.

Qualifications

• Current Louisiana LPN license required.
• Minimum of 2-3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver's license and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care under the RN and Plan of Care.
• Communicates with attending physicians and hospice team members as needed.
• Documents services and submits forms.
• Demonstrates knowledge of Medicare/Medicaid hospice benefits.
• Reports changes in patient condition and needs to RN.
• Exhibits strong time management skills.

Job duties

• Document all services provided and submit them on appropriate forms
• Demonstrates knowledge of MedicareMedicaid regulations for hospice benefits
• Report changes in patient condition and needs to RN
