Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) for full-time home health care in Williams, AZ. The LPN administers skilled nursing care under RN supervision in clients' residences and coordinates with the care team, patient/family, and referring agencies.

Qualifications include an accredited LPN program graduate, active LPN license, valid state driver’s license, and CPR/Hepatitis proof. Two years of LPN experience, preferably in home health, is desired.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Current State's Driver's License.
• Two years’ LPN experience, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
