Job summary

Angels Care Home Health is hiring a Licensed Practical Nurse (LPN) in Holbrook, AZ to administer skilled nursing care under RN supervision in clients’ homes. You will coordinate care with the interdisciplinary team, patient and family, and referring agencies.

Requirements include an accredited practical nursing program graduate, active LPN license, CPR and Hepatitis profile, and at least two years of LPN experience (home health preferred).

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Two years' experience as an LPN; home health preferred.
• CPR certification and Hepatitis profile current.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
