Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure high-quality in-home care.

The ideal candidate has an accredited LPN program, current state license, and 2 years of LPN experience (home health preferred). A valid driver’s license and CPR/Hepatitis documentation are required.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Valid driver’s license.
• Two years’ LPN experience, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under supervision of a registered nurse.
