Job summary

Amedisys is seeking a licensed practical nurse for full-time Day Visits in Shelton, CT. You will verify care plans with RNs, collaborate with physicians, educate patients and caregivers, monitor vital signs, and perform medication reconciliation during each visit.

Requirements include 1 year of LPN experience, an active LPN license in the appropriate state, CPR certification, and a valid driver’s license. Benefit eligibility varies by position.

Qualifications

• One year of clinical LPN experience.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.
• Current LPN license, specific to assigned state(s).

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
