Job summary

Amedisys is seeking a Licensed Practical Nurse for full-time home health visits in Savannah, GA. The role involves patient care at home, collaborating with RNs and physicians, and ensuring plan of care accuracy.

Experience as an LPN, current license in the state, CPR certification, and a valid driver's license are required. Compensation includes mileage reimbursement and a comprehensive benefits package.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the relevant state(s).
• Current CPR certification.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment for treatments using aseptic technique.
• Complete documentation timely and accurately.
