Job summary

Amedisys, a leading home health and hospice company, is seeking a Full-Time Licensed Practical Nurse in Nashville. Work Mon–Fri, 8 AM–5 PM, delivering patient-centered home care with strong team collaboration to support healing in the home setting.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, valid for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify if condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
