Job summary

Amedisys Home Health is hiring a Licensed Practical Nurse for our Cookeville, TN area. This full-time role operates Monday–Friday, 8AM–5PM, delivering skilled nursing care to patients in their homes.

You will verify care plans, collaborate with physicians and RNs, educate patients and caregivers, monitor vitals, and ensure accurate documentation. Qualifications

• One year of clinical LPN experience is required.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Educate patients and caregivers on self-care techniques.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
