Amedisys Home Health in the United States is seeking a licensed practical nurse to join our team providing in-home patient care. The role offers full-time hours, mileage reimbursement, and flexible options while delivering high-quality home-based care.

Responsibilities include care plan verification, patient education, vital signs monitoring, and medication reconciliation. Requires an active LPN license, CPR, and reliable transportation. Benefits include health plans, PTO, and retirement options.

Mansioni

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.

• $32-$38 Base Rate (Converting to Per Visit)
