Job summary

Amedisys in Virginia is seeking a licensed practical nurse for full-time home health care in Midlothian. You will verify plan of care with an RN prior to each visit and support patients in their home environment.

Responsibilities include medication reconciliation, vital signs, patient teaching, and coordinating with physicians. Requires 1 year LPN experience, current LPN license, CPR, and valid driver’s license. Competitive pay and benefits offered.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment for treatments using aseptic technique.
• Complete documentation timely and accurately.
