Job summary

Amedisys Home Health in Indiana is seeking a licensed practical nurse for a full-time daytime role. You will work in patients' homes, coordinating with RNs and physicians to ensure quality care and safe, effective treatment.

Responsibilities include validating care plans, monitoring vitals, managing medications, and performing physician-ordered procedures. A valid Indiana LPN license and CPR certification are required, with driving responsibilities and benefits offered.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurate.
