Job summary

Amedisys is hiring a Licensed Practical Nurse to deliver in-home care across assigned geographies. You will verify care plans with RNs, coordinate with physicians, and educate patients and families on self-care. The role requires patient-centric communication and clinical proficiency in home settings.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state-specific.
• Current CPR certification.
• Valid driver's license and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs; notify changes in condition.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

• $32-$38 Base Rate (Converting to Per Visit)
