Job summary

Amedisys, Inc. in Hinesville, GA is seeking a licensed practical nurse for full-time home health visits. You will verify the plan of care with the RN before each visit and collaborate with physicians to meet patient needs.

You will teach patients, monitor vital signs, reconcile medications, and prepare equipment for treatments. A valid GA LPN license, CPR certification, a driver’s license and reliable transportation are required.

Qualifications

• One year of clinical LPN experience required.
• Current LPN license for the state you are assigned to work.
• Current CPR certification required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify promptly if condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform medication reconciliation.
• Perform specialized procedures ordered by physician (e.g., foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
