Job summary

Amedisys is hiring a Licensed Practical Nurse (LPN) in Salem, Oregon to deliver in-home patient care. You will verify plans of care, coordinate with RNs and physicians, teach self-care, check vitals, and perform medications reconciliation and basic procedures.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state you are assigned.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
