Job summary

Angels Care Home Health is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing services in patients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patients, families, and referral sources.

The role requires an LPN license, CPR and Hepatitis profile, and a valid driver’s license, with home health experience preferred. We offer comprehensive benefits and opportunities for growth across locations.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Current state's driver’s license.
• Two years’ experience as LPN, home health preferred.
• Proof of CPR and Hepatitis profile.

• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
