Job summary

Angels Care Home Health seeks a Licensed Practical Nurse (LPN) to deliver skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate with the interdisciplinary team and referring agencies to ensure high-quality patient care.

Ideal candidates have an LPN license, a valid driver's license, CPR and Hepatitis profile, and at least two years of LPN experience, preferably in home health.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license.
• Current state's driver's license.
• Two years of LPN experience, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
• Provide care in clients' homes.
