Job summary

Angels Care Home Health seeks an LPN to provide skilled nursing care in clients' homes under RN supervision. You will coordinate with the care team, patients, families, and referral agencies to ensure quality outcomes.

Qualified candidates hold an LPN license in the state, have a valid driver’s license, and possess CPR certification with Hepatitis profile. Two years of LPN experience, preferably in home health, are desired.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN license in the state.
• Two years of LPN experience; home health preferred.
• CPR certification and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
