Job summary

Center-At-Park-Wes is seeking a dedicated LPN to join our nursing team in providing high-quality resident care under physician direction. You will perform direct nursing tasks, monitor vitals, administer medications, and contribute to care plans in a fast-paced, supportive environment.

The ideal candidate holds a current LPN license, demonstrates sound judgment, and communicates effectively with residents, families, and staff.

Qualifications

• Current, unencumbered LPN license.
• Ability to make independent decisions and exercise sound judgment.
• Strong communication and interpersonal skills with residents, families, and staff.
• Knowledge of nursing practices, procedures, and long-term care regulations.
• Ability to lift, push, pull, and transport residents with assistance.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
• Observe, assess, and report changes in resident condition to physicians and the nursing team.
• Assist in development and modification of resident care plans.
• Document resident care accurately in electronic health records.
• Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
• Participate in facility surveys and quality improvement initiatives.
• Maintain a safe and therapeutic environment for residents.
