Job summary

Amedisys is seeking a licensed practical nurse to deliver patient care in home settings. The role requires verifying plans of care with the RN, collaborating with physicians, and educating patients and caregivers on self-care.

Qualifications

• One year of clinical LPN experience.
• Current LPN license (state(s) assigned).
• Current CPR certification.

Responsibilities

• Collaborate with physician and RN to ensure patient needs are met and notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform med reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
