Job summary

Amedisys Home Health in Georgia is seeking a licensed practical nurse to provide in-home patient care on a full‑time basis. You will verify plans of care with the RN, monitor patient conditions, and collaborate with physicians to meet patient needs.

The role includes mileage reimbursement, competitive per-visit pay, and a comprehensive benefits package with PTO and health plans. Valid LPN license and CPR are required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license valid for assigned state(s).
• Current CPR certification.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met and notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
