JAG Physical Therapy in Jersey City, NJ is seeking a licensed Physical Therapist to join our care-first outpatient team. You will manage evaluations, plan and implement treatment, document progress, and collaborate with an interdisciplinary group to optimize patient outcomes.

The role includes EMR documentation, daily care, and a focus on high-quality patient education. We offer a 4-day work week option, relocation and sign-on bonuses, and a strong benefits package.

Qualifications

• Bachelor’s or Master’s degree required; Doctorate preferred.
• Licensed Physical Therapist in NJ; CPR license.
• Completed CEU requirements for NY/NJ.
• EMR software experience preferred.
• Strong teamwork and professional demeanor.

Responsibilities

• Collects and records patient care data per department guidelines and payer requirements.
• Includes initial evaluations, daily notes, progress reports, discharge plans, and team communication.
• Record daily billing and follow productivity guidelines.
• Prepare evaluations per departmental standards and assess patient diagnoses.
• Develop treatment plans and collaborate with the interdisciplinary team.
• Refer patients to other providers as needed and manage discharge planning.
• Provide patient education and caregiver training and support.
