South Pacific Rehab Services in Santa Rosa, CA is hiring an Occupational Therapist (PT) to join our growing team. You will perform comprehensive OT evaluations, develop and implement individual treatment plans, document findings, and ensure MD orders for all steps of care.

Qualifications

• Licensed and/or registered Occupational Therapist in-state.
• Degree in Occupational Therapy (B.S./M.A./M.S. or Doctorate) recognized by AOTA.
• NBCOT designation (OTR) if state mandates.

Responsibilities

• Conduct comprehensive OT evaluations based on MD orders.
• Document findings on standardized forms timely and accurately.
• Develop comprehensive treatment plans with short- and long-term goals, frequency, duration, and modalities.
• Ensure MD orders for evaluations, treatments, recertifications and discharges.
• Provide treatment to patients using appropriate modalities and adjust as needed while following precautions.
