Job summary

Outfield Healthcare Partners is seeking a licensed social worker to assess residents, develop care plans, and coordinate services in a skilled nursing setting. You will document in accordance with federal/state regulations and assist in discharge planning and family education.

The role requires a social work license, a relevant bachelor’s degree, and experience with geriatrics; nursing background is a plus.

Qualifications

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field, including Sociology, Special Education, Rehabilitation Counseling and Psychology, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident.
• Uses the Minimum Data Set (MDS) to guide the care plan.
• Maintains accurate documentation in compliance with regulations and center policies.
• Supports resident self-determination and documents refusals or alternatives.
• Participates in the development of an interdisciplinary care plan.
• Provides therapeutic interventions for adjustment to long-term care.
• Coordinates discharge planning and referrals to home care services.
• Promotes resident rights and addresses abuse as mandated by law.
• Collaborates with families and staff to support self-advocacy and quality of life.
• Assists in planning, developing, and directing social service programs.
• Supports inspection readiness and corrective actions with nursing/admin.
