Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial, mental, and emotional needs of residents in a skilled nursing facility. The role involves assessments, care planning, and coordinating discharge planning with an interdisciplinary team.

The candidate should have a Bachelor’s in Social Work or Human Services, a state license, and minimum experience in healthcare settings with elderly populations. Nursing experience in SNF is preferred.

Qualifications

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field, including Sociology, Special Education, Rehabilitation Counseling and Psychology, required.
• One year of supervised social work experience in a healthcare setting with geriatric residents.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
• Maintains accurate documentation compliant with federal/state regulations and facility policies.
• Supports resident self-determination and documents refusals with education on alternatives.
• Develops interdisciplinary care plans addressing psychosocial needs and interventions.
• Provides therapeutic support for transition and adjustment to long-term care.
• Educates residents/families and refers to social service agencies as needed.
• Coordinates discharge planning and referrals for home care services.
• Collaborates with the interdisciplinary team to protect resident rights and well-being.
• Assists in educating families and staff to promote proactive self-advocacy.
• Assists in planning, developing, and evaluating social service programs.

Education

Bachelor's Degree in Social Work or Human Services

Licensed Social Worker/Certified in state

One year supervised social work experience

Nursing experience in SNF (2+ years preferred)
