Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial, mental, and emotional needs of residents and to assist with access to services in a NASW-aligned setting. The role emphasizes ethical practice, regulatory compliance, and holistic resident support in a skilled nursing facility.

The successful candidate will have a Bachelor’s degree in social work and supervised experience in geriatric healthcare, with preferred nursing facility background.

Qualifications

• Certified, licensed, or registered in the state of practice.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
• Maintains accurate documentation in accordance with federal/state regulations and facility policies.
• Supports resident rights and informs about therapeutic alternatives to refused services.
• Participates in developing an interdisciplinary plan of care addressing psychosocial needs and goals.
• Provides psychosocial interventions to support residents’ adjustment to long-term care.
• Coordinates discharge planning and referrals for home care services as needed.
• Works with the team to educate residents and families about facility placement and services.
