Qualifications

• Master’s degree in social work or equivalent.
• Current licensure or certification as a Social Worker in the state of employment.
• Previous experience in Long-Term Care, geriatrics, or related field.
• Strong skills in assessment, counseling, and crisis intervention.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively as part of a multidisciplinary team.
• Empathy, compassion, and commitment to person-centered care.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing social, emotional, and psychological concerns.
• Provide counseling and emotional support to residents and families, guiding coping strategies and adjustment to care transitions.
• Advocate for residents' rights and preferences in care planning and decision-making.
• Connect residents and families with community resources, support services, and advocacy organizations.
• Provide crisis intervention and support during challenging situations such as grief, loss, or health changes.
