Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial, mental and emotional needs of residents in a skilled nursing facility, and to aid in access to services in line with NASW code of ethics. Responsibilities include completing social histories, maintaining regulatory documentation, supporting resident self-determination, and coordinating interdisciplinary care plans and discharge planning to enhance quality of life.

Qualifications

• Must be licensed, registered, or certified in the state of practice.
• Bachelor’s Degree in Social Work or related field from an accredited school.
• At least one year of supervised social work experience in a healthcare setting with geriatric residents.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessments identifying social, emotional, and psychological needs; uses MDS outcomes to guide care plan.
• Maintains accurate documentation in accordance with federal/state regulations and facility policy, including directives and care plans.
• Supports resident self-determination; documents refusals and educates on alternative options.
• Develops interdisciplinary care plans addressing psychosocial needs and goals.
• Provides therapeutic support for residents’ transition and adjustment to long-term care.
• Coordinates discharge planning and referrals for home care services prior to community return.
• Works with the team to protect resident rights and well-being and to address abuse as required by law.
• Assists families and residents with information and education about placement and services.
