Job summary

Outfield Healthcare Partners seeks a qualified social worker to identify psychosocial, mental, and emotional needs and facilitate access to services for residents in a skilled nursing setting.

Responsibilities include assessments, care planning, documentation, discharge coordination, and inter-disciplinary collaboration to support resident rights and well-being. Licensure and relevant experience are required, with nursing background preferred.

Qualifications

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or Human Services field from an accredited school.
• One year of supervised social work experience in a healthcare setting with geriatric clients.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying needs.
• Maintains accurate documentation compliant with regulations and center policies.
• Supports resident self-determination and educates on alternatives when declining services.
• Participates in written interdisciplinary care planning addressing psychosocial needs.
• Provides therapeutic interventions to aid transition and adjustment to long-term care.
• Coordinates discharge planning and referrals to home care services when needed.
• Collaborates with the inter-disciplinary team to protect resident rights and well-being.
• Assists in education for families and staff to support quality of life; follows standards.
