Job summary

Outfield Healthcare Partners in Dallas, Texas is seeking a licensed social worker to assess psychosocial, mental and emotional needs and help residents access essential services in a long-term care setting.

You will complete social histories and assessments, document to federal and state regulations, support resident rights, participate in an interdisciplinary care plan, and coordinate discharge planning to smooth transitions back to the community.

Qualifications

• Licensed, certified, or registered in the state of practice is required.
• Bachelor’s Degree in Social Work or related Human Services field is required.
• One year of supervised social work experience in a healthcare setting with geriatric residents is required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility is preferred.

Responsibilities

• Complete social history and psychosocial assessments identifying social, emotional, and psychological needs.
• Maintain accurate documentation compliant with federal/state regulations and company policy.
• Support resident rights and inform about therapeutic alternatives when services are refused.
• Develop written interdisciplinary care plans addressing psychosocial needs and interventions.
• Provide therapeutic support to residents adjusting to long-term care and facilitate coping strategies.
• Educate and support residents and families, referring to appropriate social service agencies as needed.
• Coordinate discharge planning and arrange referrals for home care services.
• Collaborate with the interdisciplinary team to promote resident well-being and protect resident rights.
