Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial, mental, and emotional needs of residents in a skilled nursing facility. You will assess residents, document in the medical record, and help develop an interdisciplinary plan of care addressing psychosocial needs and goals.

Qualifications

• Certified, licensed, or registered in the state of practice is required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
• Maintains accurate documentation compliant with federal/state regulations and center policies.
• Supports resident self-determination and documents decisions and alternatives.
• Develops an interdisciplinary plan of care addressing psychosocial needs.
• Provides therapeutic interventions to support adjustment to long-term care.
• Educates residents and families about placement and available services.
• Coordinates discharge planning and referrals for home care services.
• Collaborates with the interdisciplinary team to protect resident rights and well-being.
• Promotes proactive resident and family engagement in care decisions.
• Assists Social Service Director in planning and evaluating social service programs.
• Supports inspection readiness and adherence to standards; addresses survey findings.
• Other duties may be assigned as needed.
