Mass General Brigham Community Physicians, Inc. is seeking a LICSW to join a High-Risk, High-Cost Complex Care Management program in Boston. You will lead behavioral health integration within an interdisciplinary team and travel to practice sites, EDs, and patient homes as needed.

The role emphasizes crisis intervention, SDOH navigation, and collaboration with RNs, pharmacists, and care coordinators to prevent unnecessary hospital visits and manage complex cases in the community.

Qualifications

• Bachelor’s degree in social work and active LICSW/LCSW/LMHC licensure required.
• LICSW strongly preferred; 1+ year medical social work experience.
• Trauma-informed care, crisis intervention, and SDOH navigation expertise.
• Familiarity with Medicaid/Medicare care models and HEDIS/NCQA measures.
• Experience with Epic or similar EHR systems.

Responsibilities

• Conduct comprehensive behavioral health assessments and plan integrated care.
• Provide brief evidence-based interventions and psychoeducation for stress/anxiety.
• Identify social barriers via SDOH assessments and community resources.
• Reduce preventable ED visits and readmissions through proactive outreach.
• Coordinate with PCPs, behavioral health specialists, and care coordinators.
• Perform post-discharge outreach within 24–48 hours to support recovery.
