Mass General Brigham Community Physicians, Inc. seeks a Licensed Clinical Social Worker (LICSW) to join the High‑Risk, High‑Cost Complex Care Management Program, collaborating with an interdisciplinary team to prevent unnecessary ED visits and hospitalizations.

The role requires conducting behavioral health assessments, providing brief therapies, and addressing SDOH within a community-based care model across multiple sites in the Boston area.

Qualifications

• Bachelor’s Degree in Social Work from accredited institution.
• LICSW, LCSW or LMHC licensure; LICSW strongly preferred.
• 1+ years of medical social work experience.

Responsibilities

• Conducts comprehensive behavioral health assessments using validated tools such as PHQ-9, GAD-7, CAGE, and ACEs.
• Provides brief, evidence-based therapeutic interventions (MI, CBT-informed, Solution-Focused).
• Performs comprehensive SDOH assessments and interventions.
• Identifies high-risk patients using real-time ED alerts, hospital discharge data, and predictive analytics for early intervention.
• Focuses on crisis prevention and de-escalation with same-day or next-day behavioral health support.
• Collaborates with PCPs, behavioral health specialists, urgent care clinics, and community programs.
• Provides post-hospitalization outreach within 24–48 hours.
• Travels to practice sites and patient homes; hybrid role with onsite four days a week.
