Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families with social, emotional, interpersonal and environmental issues.
• Formulate biopsychosocial assessments, dispositions, and treatment plans.
• Collaborate with healthcare team to evaluate medical/physical condition and client needs.
• Monitor and document client progress toward measurable goals.
