Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Social Worker to perform psychosocial assessments and develop integrated treatment plans for inpatients, outpatients, and emergency department patients.

You will work as part of a multidisciplinary team to support patient-centered care, coordinate with providers, and document progress, with a per diem schedule and hybrid work model across community sites in the Boston area.

Qualifications

• Master's degree in Mental Health & Behavioral Medicine or Social Work is required.
• Licensure as LICSW, LMHC, or LCSW is required.
• Minimum 2 years of clinical experience in a medical setting.
• Class D Driver's License is required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop biopsychosocial disposition and treatment plans.
• Collaborate with the interdisciplinary team to evaluate medical status and client needs.
• Monitor and document client progress with measurable goals.
