Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Social Worker to perform psychosocial assessments and develop biopsychosocial treatment plans for inpatients, outpatients, and ED patients.

You will collaborate with a multidisciplinary health team to support patient-centered care and coordinate resources to meet individual needs. This role requires an MSW, LCSW/LICSW or LMHC licensure, and 2+ years of clinical medical setting experience.

Qualifications

• Master's Degree Social Work (MSW) required.
• LCSW, LICSW or LMHC licensure required.
• 2+ years clinical experience in a medical setting required.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
