Mass General Brigham Community Physicians is seeking an experienced social worker to join our multidisciplinary team in MA. The role involves psychosocial assessments, treatment planning, and collaboration with an interdisciplinary team in a hospital setting.

Qualifications include a Master's in Mental Health or Social Work and LICSW/LMHC licensure, plus at least two years of clinical experience in a medical environment. Hybrid schedule with on-site and remote work options is offered.

Qualifications

• Master's Degree Mental Health & Behavioral Medicine or Master's Degree Social Work required.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) license required.
• 2+ years of clinical experience in a medical setting required.

Responsabilités

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
