Mass General Brigham Community Physicians, Inc. is seeking a Licensed Clinical Social Worker to join the High-Risk, High-Cost Complex Care Management Program in a hybrid Boston-based role.

You will conduct behavioral health assessments, provide evidence-based interventions, and support patients across medical, behavioral, and social care domains. The role requires LICSW (or LCSW/LMHC) licensure, 1+ year of medical social work experience, and strong trauma-informed/crisis intervention skills.

Qualifications

• Bachelor’s Degree in Social Work from an accredited institution.
• LICSW strongly preferred; LCSW or LMHC may be considered.
• 1+ year of medical social work experience.
• Experience with trauma-informed care and crisis intervention.
• Familiarity with HEDIS measures and Medicaid/Medicare models.

Responsabilités

• Conduct comprehensive behavioral health assessments.
• Provide brief, evidence-based interventions like MI and CBT.
• Support substance use disorders with SDOH assessment.
• Identify high-risk patients using ED data and analytics.
• Provide crisis prevention and de-escalation with same-day support.
• Collaborate with PCPs, behavioral health specialists, and community programs.
• Coordinate post-hospitalization outreach within 24–48 hours.
• Integrate medical, behavioral, and social care across the team.
• Educate patients on substance use and mental health concerns.
• Travel to practice locations and patient homes; hybrid role four days onsite.
• Perform other duties as assigned.
