Job summary

Umpqua Health in Oregon seeks a licensed Care Coordinator to perform comprehensive care management for Medicaid/Medicare members, including LTSS. The role involves in-home assessments, planning, and collaboration with a multidisciplinary team to ensure integrated, high-quality care.

You will travel locally (25–40%), maintain strict documentation, and support members with motivation and resources while meeting compliance standards and schedule expectations.

Qualifications

• Active, unrestricted Oregon RN license; eligible to sit for CCM.
• Minimum two years healthcare experience, including LTSS.
• Care management or medical/behavioral health experience.
• Valid driver’s license and reliable transportation.
• Proficiency with Microsoft Office and navigating EHRs.
• Strong knowledge of Oregon community resources and diverse populations.
• CCM certification required within 18 months of hire.

Responsibilities

• Conduct comprehensive assessments including in-home visits.
• Develop and implement individualized care plans with members and teams.
• Monitor care plans and document interventions.
• Coordinate behavioral health, LTSS, and community resources.
• Evaluate benefits and advise on funding sources.
• Facilitate ICT meetings and team collaboration.
• Educate and support members using motivational interviewing.
• Address barriers to care and psychosocial, financial, medical concerns.
• Create prevention plans for critical incidents.
• Maintain accurate documentation in EHR and ensure compliance.
• Travel locally 25–40% for member visits; mileage reimbursed.
